When Are Palpitations Dangerous? When Should You See a Cardiologist?

Palpitations are the sensation that the heart is beating fast, strongly, irregularly, or differently from usual. Sometimes they feel like a brief “skipped beat” lasting a few seconds; sometimes they appear as rapid heartbeats lasting minutes.
Palpitations are a very common complaint and do not always mean serious heart disease. They can also arise from non-cardiac causes such as stress, lack of sleep, excess caffeine, certain medications, or anaemia.
The key question is: Which palpitations may be harmless, and which may hide an important rhythm disorder?
That distinction cannot be made from heart rate alone. How the episode starts, how long it lasts, its relation to exercise, and especially whether fainting, chest pain, or breathlessness accompany it all matter.
Quick Answer: When Can Palpitations Be Dangerous?
Palpitations often do not point to serious heart disease; they can also come from stress, lack of sleep, caffeine, or anaemia. However, if palpitations come with fainting or near-fainting, chest pain, marked breathlessness, onset during exercise, long duration, known heart disease, or a family history of unexplained sudden death at a young age, cardiology evaluation becomes more important for a possible underlying rhythm disorder.
This cannot be decided from pulse count alone. Onset pattern, duration, regular or irregular character, relation to exertion, and accompanying symptoms must be assessed together. For patients from Istanbul’s Anatolian side — especially Kozyatağı, Kadıköy, Ataşehir, and Maltepe — evaluation always starts with history, examination, ECG, and echo when needed; exercise testing and appropriately timed rhythm monitoring (Holter) may be planned according to need.
Warning features to note:
- Fainting or near-fainting
- Chest pain or pressure
- Marked breathlessness
- Palpitations starting during exercise
- Prolonged or very rapid heartbeats
- Marked dizziness or visual darkening
- Known heart disease
- Family history of sudden unexplained death at a young age
- Severe weakness, cold sweating, or low blood pressure with palpitations
These features do not prove a dangerous rhythm disorder, but they make cardiac investigation of the cause more important.
Especially if palpitations occur with fainting, ongoing chest pain, severe breathlessness, or marked deterioration in general condition, urgent medical evaluation may be needed rather than waiting for a routine clinic appointment.
What Are Palpitations?
Normally we often do not notice our heartbeat through the day. When heartbeats become clearly felt, this is generally called palpitation.
Patients describe palpitations in quite different ways:
- “My heart is racing.”
- “It feels like a bird fluttering in my chest.”
- “My heart pauses for a moment, then thumps hard.”
- “My pulse suddenly speeds up.”
- “My heart is beating irregularly.”
- “I can feel my heartbeat in my throat.”
Each description may point to a different rhythm mechanism. Asking only “how high was the pulse?” is not enough.
Is Every Palpitation a Rhythm Disorder?
No. Palpitation is a symptom, not a disease name by itself.
Heart rhythm may be entirely normal while the person simply feels beats more strongly; true rhythm disorders can also cause palpitations.
Non-cardiac contributors include stress and anxiety, lack of sleep, excess coffee or energy drinks, smoking and nicotine, alcohol, fever, dehydration, anaemia, thyroid disease, some medications, and electrolyte imbalance.
By contrast, extrasystoles, supraventricular tachycardias, atrial fibrillation, and some ventricular rhythm disorders can also cause palpitations.
The aim is not to fear every palpitation, but not to miss episodes with high-risk features.
Which Features Matter More With Palpitations?
1. Fainting With Palpitations
Fainting at the same time as palpitations deserves particular attention. Syncope has many causes, and many are non-cardiac or benign. However, very fast or very slow rhythm disorders can briefly reduce brain blood flow and cause loss of consciousness.
The 2018 ESC Syncope Guidelines emphasise that syncope related to exertion or occurring on a background of structural heart disease needs more detailed cardiac evaluation.
Fainting right after palpitations, sudden loss of consciousness without warning, syncope during exercise, or syncope in someone with known structural heart disease warrants more detailed cardiology assessment.
2. Palpitations During Exercise
An increase in heart rate with exercise is normal. However, sudden marked palpitations while walking, running, exercising, or climbing stairs — especially with dizziness, chest pain, breathlessness, or near-fainting — should be evaluated.
What matters is not only a fast heart, but the relation to exertion and accompanying symptoms.
3. Chest Pain With Palpitations
Pain or pressure in the chest during palpitations can have different causes. Very rapid rhythms increase the heart’s oxygen demand. In people at risk for coronary artery disease, chest pain also needs separate assessment.
New or severe chest pain with palpitations — especially if prolonged or accompanied by sweating, breathlessness, or feeling unwell — may require urgent evaluation.
4. Breathlessness With Palpitations
Mild shortness of breath can occur with some brief episodes. Marked breathlessness, especially with a prolonged rapid rhythm, needs more caution.
A very fast or irregular rhythm can make it harder for the heart to maintain circulation effectively in some people. This is more important in known heart failure or structural heart disease.
5. Sudden Onset and Sudden Offset
Some patients describe: “It started suddenly as if a switch was flipped, raced for a few minutes, then stopped abruptly.”
This pattern can be seen in some supraventricular tachycardias. Many such episodes can be managed with appropriate evaluation and treatment, but identifying the rhythm behind recurring attacks matters.
An ECG recorded during an attack is therefore highly valuable. If the episode is short and gone before reaching care, longer rhythm-recording methods may be considered.
6. A Very Irregular Pulse Sensation
Palpitations are not always fast and regular. Some people say “My heart is completely irregular.” Causes range from simple extrasystoles to clinically important disorders such as atrial fibrillation.
The 2024 ESC Atrial Fibrillation Guidelines emphasise recognising and appropriately evaluating atrial fibrillation, especially in older adults and those with stroke risk factors.
New irregular rhythm is particularly important to evaluate in older age, hypertension, diabetes, heart failure, or prior stroke.
Does a Family History of Sudden Death Make Palpitations More Important?
Yes — in some situations family history clearly changes risk assessment. Unexpected or unexplained sudden death at a young age in the family should always be mentioned in a cardiology evaluation.
The 2022 ESC Guidelines on Ventricular Arrhythmias and Prevention of Sudden Cardiac Death emphasise family history in investigating inherited rhythm diseases and cardiomyopathies.
Young sudden death, unexplained drowning, single-vehicle crash, cardiomyopathy, or known inherited arrhythmia in the family can change the evaluation approach.
In these people, assess not only the palpitations themselves but personal and family risk together.
Are Palpitations More Important in Someone With Heart Disease?
They do not always mean danger, but they are assessed more carefully.
After heart attack, in heart failure, cardiomyopathy, significant valve disease, congenital heart disease, or prior serious arrhythmia, new palpitations may mean something different than in a structurally normal heart.
What matters is not only finding a rhythm disorder, but understanding on which heart it occurs.
What Is Assessed in a Cardiology Visit for Palpitations?
The first aim is not to attach a Holter or order many tests. The first step is usually a detailed history.
Especially important questions include:
- When did palpitations start?
- Sudden or gradual onset?
- How long do they last?
- Regular or irregular?
- Do they occur with exertion?
- At rest or at night?
- Fainting or dizziness?
- Chest pain or breathlessness?
- Was pulse measured during an attack?
- Medications or stimulants?
- Family history of sudden death or rhythm disease?
These details sometimes guide test selection more than the test itself.
Which Tests May Be Used for Palpitations?
The same tests are not appropriate for every person with palpitations.
ECG
A resting ECG is often among the first assessments. If recorded during an attack it may show the diagnosis directly. If palpitations are intermittent, the ECG in clinic may be entirely normal. A normal ECG does not exclude all intermittent rhythm disorders.
Echocardiography (Echo)
Echocardiography (echo) is used to assess heart structure, not to record the rhythm. It provides information on pumping function, chambers, valves, and some structural diseases. It is not mandatory for every palpitation; it is planned according to clinical evaluation.
Holter and Longer Rhythm Recording
If palpitations are not caught in clinic or on a standard ECG, longer rhythm recording may be needed. 24–48-hour Holter may suffice for some; rarer attacks may need longer methods.
The core principle: recording duration should match how often symptoms occur. Someone with weekly episodes and someone with several daily episodes may not need the same monitoring approach.

Does a Smartwatch Finding of Palpitations Make the Diagnosis?
Smartwatches and personal rhythm devices have become more common. A good-quality recording during an attack can be genuinely useful for a cardiologist. Capturing onset and offset can provide important information.
However, a smartwatch does not replace a cardiology consultation or a medical ECG. Device alerts for “atrial fibrillation,” “high pulse,” or “irregular rhythm” should be interpreted with clinical findings and medical recordings when needed.
When Should You Go to the Emergency Department for Palpitations?
Urgent medical evaluation may be needed if any of the following occur while palpitations continue:
- Fainting or loss of consciousness
- Severe or ongoing chest pain
- Marked breathlessness
- Severe weakness or deterioration in general condition
- Cold sweating and feeling unwell
- Markedly low blood pressure
- New neurological symptoms
If symptoms continue, seeking appropriate emergency care is safer than trying to drive a long distance oneself.
If Palpitations Have Stopped, Is a Cardiologist Still Needed?
In some situations, yes. That the episode has ended by the time of assessment does not mean the cause is unimportant. Many rhythm disorders occur in attacks and may have returned to normal by the time the patient reaches a doctor.
Evaluation is appropriate especially if attacks recur, become more frequent, last long, occur with exertion, come with fainting or dizziness, if known heart disease is present, or if there is a family history of sudden death.
The Right Approach: Assess Risk First
The aim is not the same tests for everyone. One person may have a few seconds of clinically low-risk extrasystoles; another may have significant tachycardia under the same word “palpitations.”
The right sequence is: listen to the complaint → identify high-risk features → examination and ECG → choose appropriate further testing only when needed.

Palpitations are very common, and most alone do not mean serious heart disease.
However, palpitations with fainting, onset during exertion, chest pain, marked breathlessness, known heart disease, or family history of sudden death at a young age make evaluation more important.
The goal is not advanced testing for every episode, but investigating potentially important rhythm disorders in the right patient with the right method.

No. Palpitations are common and can also come from stress, lack of sleep, excess caffeine, anaemia, thyroid disease, or certain medications. In people without structural heart disease, some isolated extrasystoles may be clinically benign and not always need treatment. At what pulse rate are palpitations dangerous?
It is not correct to separate “dangerous” from “safe” by a single pulse number alone. Age, rhythm type, duration, whether heart disease is present, and accompanying symptoms must be assessed together. Mild resting rate increase and a sudden very rapid rhythm lasting minutes can mean different things clinically. Evaluation is based on the full clinical context, not pulse count alone.
Is dizziness with palpitations important?
It can be. Marked dizziness, visual darkening, near-fainting, or loss of consciousness occurring with palpitations makes cardiology evaluation more important. Some rhythm disorders can cause these symptoms when the heart cannot maintain circulation adequately. If this combination occurs, evaluation should not be delayed.
Are nighttime palpitations dangerous?
Nighttime occurrence alone does not mean they are dangerous; many people feel heartbeats more clearly at rest. However, if they recur often, last long, wake you from sleep, or come with other symptoms, investigating the cause is appropriate. A rhythm recording during an attack can help evaluation.
If the ECG is normal during palpitations, does that mean there is no problem?
An ECG recorded while palpitations are truly ongoing is especially valuable and may show the diagnosis directly. A normal ECG after symptoms have stopped does not fully exclude intermittent rhythm disorders. Longer rhythm monitoring may be needed when attacks are infrequent. A normal resting ECG alone does not mean the cause of palpitations is fully explained.
How many hours of Holter are needed for palpitations?
No single duration is right for everyone; recording length follows how often attacks occur. Frequent daily palpitations may be covered by 24–48-hour Holter, while weekly or rarer episodes may need longer methods. The aim is to match recording duration to symptom frequency so rhythm can be captured during symptoms.
Is echo necessary for palpitations?
Not for every patient. Echo assesses heart structure and function, not rhythm recording. Echo may be planned when structural heart disease is suspected, examination or ECG is abnormal, or there is known heart disease. Clinical evaluation decides who needs it.
What should be done if palpitations and fainting occur together?
Loss of consciousness at the same time as palpitations should be evaluated, because some very fast or very slow rhythms can temporarily affect circulation. If symptoms continue, or chest pain, severe breathlessness, or deterioration in general condition accompany them, emergency care may be needed. After fainting resolves, detailed history, examination, and ECG remain important to identify the underlying cause.
